More info

  • How to make a referral

Service users are referred into this service via a referral form completed by a GP, School, Social Worker or any other professional who is involved in the young persons’ care and deems a referral to Child and Adolescent Mental Health Services (CAMHS) to be appropriate. Service users cannot self-refer. You can download a copy of the referral form below.

It is important that the following information is included in the referral

    • Presenting concerns and their impact on functioning
    • Family background, context and history
    • Goals with referral
    • Details of support attempted in the community (e.g., school, third sector)

The above information is key to understand whether a CAMHS intervention is required and/or what other support would be useful in the community.

CAMHS considers referrals of young people whose needs will be best met in getting more help and risk support areas of Thrive.

Referral requests for Neuro developmental assessments such as Autism or ADHD require additional information from an Education setting. To avoid any delays processing this referral request please ask school to complete the following forms alongside the referral form:

    • SNAP IV
    • CAST
    • ASD Checklist

These forms are all downloadable below.
