Description
CORE stands for “Clinical Outcomes in Routine Evaluation”. The CORE system comprises tools and advice to support monitoring of change and outcomes in routine practice in psychotherapy, counselling and any other work attempting to promote psychological recovery, health and wellbeing. They have a variety of measures:
- The CORE-10: a 10-item measure asking how a person has felt over the past week. It is a session-by-session monitoring tool with items covering anxiety, depression, trauma, physical problems, functioning and risk to self. The total score indicates a persons’ level of psychological distress.
- The Young Person’s CORE (YP-CORE): a 10-item measure designed for use with the 11-18 years age range.
- LD-CORE-15 and LD-CORE-30: Two forms that have been developed for use with people with learning disabilities:
- The CORE-A: a separate information gathering framework designed to be completed by practitioners, to give context to the self-report measures. It comprises two forms; one to be completed at assessment and the other at end of therapy.
Typically, patients complete the questionnaire themselves, though practitioners can assist and will want to discuss the results. It is often sent out before an appointment or completed upon arrival, providing a quick and effective way to track mental health needs, guide treatment decisions, and monitor progress over time.

- What does it measure?
The questions in CORE measurements to understand a person’s needs associated with anxiety, depression, trauma, physical problems, functioning and risk to self.
- Who is it for?
The CORE-10 is for people aged 17 and over. The YP-CORE is for young people aged 11-18. There is also a version for people with learning disabilities.

- How do you use CORE measurement tools?
You use the CORE measurements by filling out a questionnaire, rating your mental health symptoms on a scale. Clinicians then review your responses to assess your overall well-being, track progress, and make treatment decisions. The number of items on the measurement will depend on which version you are filling out.
There are a range of questions which aim to understand how a person has been feeling in the past week. Some examples include ‘I have felt tense, anxious or nervous’ and ‘talking to people has felt too much for me’.
These statements are answered by a 5-point scale, ranging from ‘Not at all’ to ‘Most or all of the time’
- Who completes this measure?
The CORE is completed by the service user, and a practitioner can offer support to help completion. Completed forms should always be discussed.
The form should be sent out before an appointment to be brought back to an appointment. If it isn’t sent out, you will be asked to complete it when you arrive for an appointment.
You will be asked to complete a CORE measurement at your first appointment and then at regular intervals during your intervention.
